Parent Service Form

Takoma Academy Preparatory School is excited to partner with our parents to foster and build relationships within our school community. Each family is required to complete, at minimum, 15 service hours by April 30th of each year or pay a family fee of $250 to be charged on May 1st of the same year. Forms submitted after April 30th will not be accepted.

After volunteering for each activity or event, please complete this form. Once submitted, the form will be automatically sent to the activity coordinator. The coordinator will verify the information and submit to the office. If the information provided is not accurate, the coordinator will return the form to you to resubmit with the accurate information before submitting to the office.
